Almost always the flapper or the fill valve. The flapper is a rubber seal that hardens and stops sealing the tank, so water trickles into the bowl and the fill valve keeps topping up. Both are inexpensive parts and a short visit. Left alone, a running toilet wastes more water than any other fixture in the house.
Treat it as urgent. It usually means the wax seal between the pan and the flange has failed, which means water — and waste — is escaping under the toilet and into the subfloor every flush. The repair itself is routine. The floor damage from ignoring it for a year is not.
